How do I make a referral to Headwise?
Please fill out a relerral form. A copy of the referral form can be sent to you upon request. You can also make a referral through our website at headwise.ca.

I have a client whom I think is suited for a Neuropsychological Vocational assessment, but I am not sure. What should I do?

What should I tell the client about the assessment?
The assessment takes place over the course of one to two days, and consists of an interview followed by cognitive activities. we try to complete testing in once day to make the process most accessible to clients, but on average the process does take 1.5 days to complete. We therefore book two full days, just in case. Clients are provided with a lunch break, along with any other break they might need. Please remind the client to bring reading glasses, hearing aids and/or medication, a lunch or money for lunch, and a copy of their resume (if they did not provide you one to pass on to us beforehand). The assessment is done entirely on at a desk, so there is no physical activity and they should dress confortably.
How long before the report is ready?
The report will be sent out to you in roughly 4 weeks from the time of the clients last appointment.
What happend if my client started an assessment but did not complete it for some reason (e.g. declined completing assessment)?
If a client started an assessment but did not complete it, we will notify you of the situation. You may decide to ask the clinician to prepare a partial report based on the information we were able to gather. In such cases we will charge a portion of the normal cost, which would depend on how much information has already been gathered.
Who do I contact to set up a debriefing/feedback session?
Our offoce manager, Lily, will contact you once the client has completed the assessment once the client has completed the assessment to set up an appointment. You can also contact us directly at 604-417-7390 or by email at firstname.lastname@example.org.
Can the assessment take place at our office?
Yes, our staff travel out to see clients at the referral sources office. However, if space is limited or unavailable, we can assess the client in one of our offices.
Is there a specific type of office space that is needed?
We would require office space that is in a relatively quite area, with a desk set up so that our staff and the client can sit acoss from eachother.
If the assessment took place at a Headwise office, does the feedback session also occur there?
No, our clinician will travel to you office to meet with you can the client for the feedback session. A telephone or teleconference (e.g. Skype) feedback option is also available; this would envolve you and the client meeting at your office, and our clincian calling in to your office at the time of the appointment.
How long are feedback sessions?
They are one hour long.
